Among the EMRs used over the past 15 years eClinicalWorks stands out as the best leaving a lasting impression. It's user-friendly and the customer support addressing most concerns within 30 minutes is exceptional and uncommon in the industry.
Cons

“Absolutely awful in every way”

Honestly there's nothing positive I can point to as this product is awful in nearly every way. We made the regrettable choice to switch our EMR to cut costs and believe me, any money we thought we saved was more than wiped out many times over by the massive drop in productivity and constant hassles. The overly rosy reviews here strike me as suspicious and feel completely fake.
Cons
I have too many complaints to list briefly but to start the interface looks ancient and the visual design is poor, with no clear button boundaries making it easy to tap the wrong thing. The whole system is slow and unresponsive, freezing and hanging regularly. And it is so slow during clinic hours that I cannot chart in real time and end up doing notes at night, only to have servers go down frequently. It feels like they don't invest in adequate server infrastructure nas individual clicks can take 10 to 20 seconds. Beyond performance, there is a credibility problem. It has been sued and fined by the federal government over reliability issues tied to a patient death and now the software displays alarming, almost defensive prompts like check copied information for completeness when using templates which implies the program itself didn't do its job or check med interactions when severity is major which makes me wonder if mild or moderate interactions won't even get proper checking. Migration from Athena was a disaster for us as nearly nothing transferred correctly. Charts came over with missing data such as labs, imaging and vaccines didn't migrate so the whole switch felt like a complete waste of time and money and the company's promises about migration were misleading. The app is full of baffling acronyms such as DI for diagnostic imaging, TInj for therapeutic injection and others like CDSS or DRTLA that I still do not fully understand and frankly do not care to memorize. There is no simple, quick way to see a patient's active problems and current medications in one place. As that information is scattered across multiple sections and requires several clicks to piece together so I waste one to two minutes at every visit trying to figure out why the patient is here and what meds they take. Menus are repetitious and the same demographic or patient details are shown repeatedly in different parts of the screen. The click burden starts at login such as type username, click, it spawns a new window, then type password clicks spawn more windows and I have seen up to five windows for a single patient, none of which can be moved or resized. Tasks must apparently be completed within about 20 seconds of opening a window or the system will falsely claim another user was in the chart and refuse to save which is both infuriating and dishonest. So when prescribing I often have to close multiple windows, go look at labs and then try the order again. Lab comparisons are painful too as to trend values I have to close windows and click a different date instead of having a simple trend view. Data loss is a real safety issue as the EMR has dropped patient information, medications disappear from lists and when reordering it has stripped doses and instructions. HPI details disappear frequently, pharmacy requests vanish and outside messages get lost. The search functionality is also bad as no smart search so prescribing is maddeningly inefficient. As you type a medication name, close the window, then reopen to specify dosing. You cannot even prescribe two entries of the same medication in one encounter if doses differ. With certain updates, NDC codes are removed making prescriptions impossible. Default med settings are incorrect. For example, metformin is set to once daily rather than twice and there's no prepopulated quantity for topical meds like there was in Athena. The workflow forces you to enter a diagnosis before meds or labs and the diagnosis defaults to whatever is first on the list so if you need to change it you must delete the order and recreate it under the correct diagnosis. Interaction checking is overly broad and slow, scanning everything a patient was ever prescribed rather than limiting to the current encounter. Telehealth is a multi-click nightmare as search patient, click name, click hub, click new encounter, click virtual visit, click open progress note before you can even type. Support is terrible. You might get a tech whose accent makes understanding difficult. So if you're an MD considering this product, seriously just run. I would not recommend this to anyone who values time, patient safety or basic usability.

I use eClinicalWorks and in my experience its cloud-based EMR provides a full end-to-end solution as handling everything from registering patient appointments and recording progress notes to ordering medications and ultimately submitting claims.
Cons
