Careficient AMS is a cloud-based EMR and agency management system used by clinical, administrative, and billing teams in home health, hospice, and palliative care. Users often value its ease of use, though some say that it can be tricky to create custom reports. Recently, the platform added integration with CareXM to improve real-time triage coordination in post-acute settings.

Our Verdict

Careficient AMS is worth considering for established or growing care providers that manage complex regulatory requirements or several agencies. Its per-user pricing may not suit small agencies but can work well for growing operations. We recommend it for home-based care organizations seeking software built around their workflows.

What Is Careficient AMS?

Careficient is a cloud-based Electronic Medical Record (EMR) software for home health, hospice, home care, and palliative care agencies. Its features include patient record management, scheduling tools, caregiver management, point of care management, billing, payroll, Electronic Visit Verification (EVV), analytics, and reporting.

These capabilities support home health and hospice agencies with requirements such as Outcome and Assessment Information Set (OASIS), Hospital Information System (HIS) submissions, payer management, authorization tracking, built-in compliance tracking, and eligibility checks. In addition to its core software, the vendor also offers services, including Revenue Cycle Management (RCM), training, and consulting.

What Is Careficient AMS Best For?

A standout aspect of Careficient is its embedded workflow automation, which is built into the EMR rather than added through separate tools. It supports tasks across referral intake, documentation, eligibility checks, medication records, and billing. Capabilities such as adaptive forms, auto-imported referrals, medication carry-forward, and real-time eligibility checks help agencies reduce repetitive administrative work across the care workflow.

How Much Does Careficient AMS Cost?

Careficient AMS pricing starts at around $995/user and can be customized per an organization’s needs. Apart from the base subscription, agencies should also consider the following estimated additional costs:

Careficient AMS Use Cases

Based on our analysis of the platform’s capabilities and user feedback, we have identified several scenarios where the software works well:

Home Health Groups Managing Multiple Agencies

Running several home health agencies can become difficult when each location relies on separate systems, often requiring administrative teams to manually consolidate data for invoicing, payroll, payment posting, and reporting. This workload can increase as the organization expands. Careficient AMS supports multiple agencies in one system and allows back-office functions to be shared across locations.

Agencies Preparing For Regulatory Surveys

Home health and hospice agencies may need to demonstrate that clinical documentation and operational processes meet State, Medicare, and accreditation requirements during surveys. Gaps or inconsistent records can create additional review work and increase the risk of findings. Careficient helps agencies keep documentation and workflows aligned with these requirements, making survey preparation more structured.

Agencies Looking For An Easier EMR Transition

Switching EMRs can disrupt daily work when staff must learn a new system while continuing patient care. This can be especially difficult when multiple roles require training before go-live. Careficient supports the transition with implementation assistance covering onboarding, training, and system setup.

Palliative Care Providers Managing Patient Transitions

Palliative care patients can later move into hospice or other care settings as their condition changes. Re-entering information during these transitions can create extra administrative work and interrupt continuity between care teams. The system supports one click patient transitions, including movement from palliative to hospice care. Patient information and care plans can also follow changes in the patient’s care location.

Is Careficient AMS Right For You?

If you run a home health, hospice, or palliative care agency that needs its EMR to exchange data across connected systems, Careficient AMS is a good fit. It supports interoperability standards, such as Health Level 7 (HL7), Fast Healthcare Interoperability Resources (FHIR), and Clinical Document Architecture (CDA), which can help agencies connect with third-party providers as their technology needs to expand without repeated data entry. On the compliance side, Careficient is a Community Health Accreditation Partner (CHAP) and Accreditation Commission for Health Care, Inc. (ACHC) certified EMR SaaS solution.

User friendly tools

Pros

Platform is very user friendly and the built in help resources are really useful so there's not always a need to contact support. When a call is necessary though the support team is very helpful.

Cons

I'd appeciate a hard stop on visit notes for "long visits" before the note is finalized rather than only getting a notification after the visit has already been locked. I actually requested this feature almost two years ago.

Easy to use

Pros

Once you get familiar with it the system is pretty simple to figure out and use.

Cons

A limitation is that when a transfer summary needs to be sent to a hospital by fax it can't be done directly through the system.

Helpful team

Pros

Whenever questions come up the team is extremely knowledgeable and always ready to help. They respond quickly answer everything in a timely way and we can count on getting a callback as well.

Cons

The only issue we've run into with the software happens when generating a care plan since a lot of the documentation from nurses notes does not carry over into the assessment or the generated plan of care.
